This file can be opening in free adobe reader 7 or above and using to type in cost and description of work or description of work codes.

This is a screen shot of the file with blocks you can fill out highlighted:

As you can see you can also add in the company or person whom you are working for name plus the work week date. This sheet can be uses as an invoice but in intended for a person whom works at home or telecommute to tracker hours for their own personal use.
